Dental Benefits

As a courtesy, we will verify your insurance and file your insurance on a non-assigned basis, therefore payment is expected at the time of service. If you have dental insurance, please bring your insurance card so that we can make a permanent copy for our records. We ask that you keep us informed of any change to your insurance because it is important that all information about you and your insurance be current.

Most insurance companies will respond within four to six weeks. Please call your insurance company if you have not received payment or an explanation of benefits within that time frame.

You may print out our 'Informed Consent Form' or our 'Authorization for Submission of Claims and Assignment of Benefits Form' and 'Authorization For Release of Health Information Form,' sign and bring it with you to your appointment.
