Our Services

Initial Consultation

Your initial visit with us will include a review of your medical and dental history, a complete examination of your mouth, and a consultation appointment. At this visit, a treatment plan will be presented to you with the goal of bringing you back toward a more healthy periodontal condition. We will notify your family dentist of the findings and recommendations.

Please assist us by providing the following information at the time of your consultation:

• X-rays from your referring dentist.
• A list of medications you are presently taking. (Please note that you can fill out your patient registration and medical history form on this web page. )
• If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
• IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
